Making a referral

There are several ways you can make a referral depending on an individual's circumstances.

 

Schools, Childrens and Families

If the person you are referring attends a school or college and is up to 18 years of age use the Essex Travel Training Referral Form.
 
 

Guidance

Make sure all sections of the referral are complete. If the referral is urgent please show this on the form.
Ensure that you give as much information as possible. Ask the parent/carer to sign and return the consent form.
 
After completion return both forms together to the address shown on
 the referral form or email to traveltraining@essex.gov.uk
Once received at County Hall the referral will be logged and issued to an appropriate trainer.
 
Travel Trainers generally have a waiting list and will make contact when a place becomes available.
 
Please contact us if you would like the referral and consent form posted or emailed to you.
 
If you need any further information regarding your referral please contact us.
 

Adult Social Care

 
 If the person you are referring is 18 years or older use a Transport Needs Assessment (TNA) request download the Transport Needs Assessment form.
 

Guidance

Make sure all sections of the form are complete
Ensure that you have given us as much information as possible
If the request is urgent please show this on the form
After completion return the form to the address shown on the form or email to

Once received at County Hall the request will be logged and issued to a Transport Needs Assessor who aim to make contact with the applicant within four weeks.
