All applicants will need to provide all of the following:
- proof of identity
- proof of address
- a recent photo of yourself
- your national insurance number
Depending which category you're applying under, additional evidence may also be required.
Your application cannot be processed if any of these items are missing.
Important: Please send copies only as we cannot return original documents to you.
Proof of identity
Any one of the following:
- a passport
- a valid driving licence (This can only be used for either proof of address or proof of ID. It cannot be used for both)
- a birth/adoption or marriage/divorce certificate
- a civil partnership/dissolution certificate
Proof of address
Any one of the following:
- a current year Council Tax statement
- a benefit or pensions letter dated within the last 12 months
- an NHS/doctor’s letter or utility bill dated in the last 3 months
- a valid driving licence (This can only be used for either proof of address or proof of ID. It cannot be used for both)
(Note: Address details held on the DWP database cannot be used to verify your current address)
Photo
This must:
- be taken within the last 6 months (we cannot accept photos that have been used on a previous Blue Badge)
- be in colour
- show the applicant's full face
- be looking straight at the camera
- have eyes open and clearly visible
Please note: Photos should be similar to passport photos, but they don’t need to be taken in a photo booth, or by a professional. You can upload a photo from your digital camera, smartphone or tablet. See guidance for taking a digital photo from the HM Passport Office.
The photo does not qualify as proof of identity.
National Insurance Number
You should also have your National Insurance Number to hand, as you will be asked for it on the form.
